Advanced Training/Spring Break Registration Procedure

Please review the following information then proceed to the bottom of this section to click on printable registration form.

Complete the PDF registration form below and return it with the appropriate participant fee(s). All fees include tax. No confirmation calls will be made. Registrants are accepted on a first registration basis. Each activity or program has limited space!
Acceptance into one session or program DOES NOT RESERVE OR GUARANTEE placement into additional sessions or programs. We recommend individuals pre-register by completing additional registration form(s) and paying session fee(s) associated with the desired program as early as possible.
IMPORTANT: Class participant are subject to a $10 Late Registration Charge if registering 1 week prior to the start of session date.
Withdrawing class participant requires a $25 processing charge. NO REFUNDS TWO weeks before start of the session dates indicated.
All participants as well as parents and guardians are required to follow the rules and policies of Break Away Sports Center, Inc. ALL ADULT OR PARENT OF YOUTH PARTICIPANT’S MUST SIGN THE PLAYER CONSENT FORM FOR THE CURRENT SEASON IN ORDER FOR THE CHILD, OR ADULT PARTICIPANT TO PLAY.
All soccer participants must wear shin guards, indoor soccer shoes, turf shoes, gym shoes or cleated shoes (some restrictions apply) to participate. Street shoes are not acceptable...no exceptions.
All INSTRUCTIONAL cancellations Due to weather/snow may require make up at the end of the winter 3 schedule and may require an alternative day. Weather/Snow make up will not be credited or refunded.
Break Away may change instructors of class without notice and does not guarantee the assignment of class instructor.
Break Away has the right to cancel class due to low enrollment before or after the 1st week for a full refund.
