Fall League Registration Procedure

Please review the following information then proceed to the bottom of this section to click on printable registration form.

League Registration Information
  1. All fees include tax
  2. Download, print, and complete the attached PDF registration form below and return it with the appropriate team or participant fee(s). No confirmation calls will be made. Team Registration after August 13 requires an additional $40 late fee (Team Fee: $645, Over 50 mini-team is $345).
  3. Unless otherwise noted, a minimum $400 NON-refundable team deposit is required and is credited to total league fee. Adult Over 50 Mini-teams are required to pay team fee in full. ALL FEES are non-refundable 2 weeks prior to the start of the session.
  4. IMPORTANT: Teams registering with partial payment MUST FILL OUT CREDIT CARD INFORMATION. ANY TEAM FEES REMAINING AFTER 1ST GAME WILL AUTOMATICALLY BE CHARGED TO THE CREDIT CARD WITHOUT NOTIFICATION.
  5. Independent players must complete a registration form with appropriate fees. A $25 administration charge is required on any refund. ALL FEES are non-refundable 2 weeks prior to the start of the session.
  6. Break Away is not responsible for forfeited games and will not reschedule games.
  7. The designated team manager is required to pick up a team packet which includes a rule book. It is the manager’s responsibility to inform team players on the rules of participation. Everyone is required to follow the rules of play determined by Break Away Sports Center.
  8. Acceptance into one session or program DOES NOT RESERVE OR GUARANTEE placement into additional sessions or programs. We recommend teams or individuals pre-register by completing additional registration form(s) and paying session fee(s) associated with the desired program as early as possible.
  9. In the event of game cancellations, make up may require an alternative day. Make up games will not be credited or refunded.
  10. Schedules are subject to change. League alignments may vary from session to session in an attempt to balance leagues. Days & times may vary from session to session.
  11. Many variables go into aligning divisions. Your skill level information, although important, does not guarantee the division that your team is placed into. Every effort will be made to equalize competition levels within the division of the league. However, Break Away does not guarantee quality of play! Break Away is NOT responsible for games forfeited.
  12. All soccer participants must wear indoor soccer shoes, turf shoes, gym shoes or cleated shoes (some restrictions apply). Street shoes are not acceptable...no exceptions.
  13. ALL PLAYERS MUST WEAR shin guards AND PROPER SHIRT COLOR TO PLAY.
